If you have decided to undergo a virtual consultation with Dr. Taylor, you must fill out the form below. It has 4 set of questions which must be answered to ensure your virtual consultation process is successful. You are also required to upload pictures of yourself clearly showing the areas of concern so that our team can get an idea about the most suitable procedures and treatments that will help you reach your desired aesthetic goals. It is advisable that the images you upload are less than 5MB each and you can upload a maximum of 10 images.
After you have submitted your virtual consultation form, a member of our team will contact you to confirm the receipt of your request. In case we need any additional information that was not collected via the form, this is when we will enquire about them. Once we have all the information required to give you the best results, we will set up your consultation.
